Zirconia or veneers? Deciding while preserving tooth structure

In aesthetic planning, in which situation is a crown preferred, and in which a porcelain veneer or bonding?

In aesthetic planning the first question is not "which material" but how much tooth structure will be sacrificed. The same appearance can be achieved with different methods; the difference between them lies in the amount of irreversible preparation and in the long-term durability of the tooth.

The least intervention first

For small fractures, narrow gaps and limited colour corrections, composite bonding is often sufficient: it requires no or very little preparation, is finished in the same session and can be repaired when needed. Its lifespan is shorter than that of porcelain and it may stain over time; but because it preserves tooth structure, it is considered as the first option.

When are veneers (porcelain laminates) used?

For colour, shape and mild alignment problems in the front teeth; porcelain 0.3–0.7 mm thick is bonded to the front surface of the tooth. As long as it stays on enamel, the bond is strong and the result is long-lasting. For this reason veneers work as expected when the tooth is largely sound.

When is a crown (zirconia / all-ceramic) used?

If the tooth already has a large filling, has had root canal treatment, is fractured or is severely discoloured, a restoration that encloses the whole tooth is needed. Here the crown removes more tissue from the tooth, but at the same time it wraps around the remaining tissue and protects it against fracture. In other words a crown is not "better"; it is the answer to a different requirement. Details: zirconia crowns.

If the alignment is poor

In marked crowding, "making the teeth look straight" with restorations means preparing sound teeth more heavily. In these cases the teeth are first put in position with orthodontics, and then, if necessary, minimal restoration is carried out. The result is both more natural and involves less tissue loss.

The trial stage: mock-up

The planned shape is tried in the mouth with a temporary material. At this stage the tooth length, midline, lip support and smile line are assessed together; if there is any objection, the design is changed before any preparation is done. Details: smile design.

Two things that protect the result

Gum health and the bite. A restoration placed over inflamed gum quickly loses its marginal fit; if there is a clenching habit, porcelain fracture is common when a night guard is not used. Aesthetic treatment is not planned until these two issues have been resolved.

Frequently asked questions

Will my teeth be cut for veneers?

In most cases preparation of 0.3–0.7 mm is made at the level of the enamel; in suitable cases they can be placed without preparation.

Do crowns change colour over time?

The ceramic surface does not take up stains. If natural teeth are whitened, the crowns do not whiten; this is why the order matters.

How often do they need to be renewed?

Well-maintained restorations are used for many years; what is decisive is gum health, the bite and clenching habits.
